Out of Town Customer Bridesmaid Measurement Form

PLEASE HELP US ORDER YOUR GOWN CORRECTLY!! Be accurate when measuring, as all gowns are ordered from the information you give us. We use your measurements and the manufacturers measurements charts to determine a size. If unusual corrections are needed due to any misinformation, you must bear the cost of any additional fabric or labor. Do not take your measurements too tightly.

If for any reason (i.e. pregnant, on a diet) you do not wish us to determine a size, you may request a size. By requesting a size, you are fully responsible for any extra costs incurred to give your dress a proper fit.

Please return this form with your deposit as soon as possible. Simply fill in the information and click the Submit button at the bottom of the page. Or, use the printable copy and mail it to us. Consult the bride for cost and deposit information. A deposit of one half of the dress total is required to order the dress. You can mail in a check or call with a credit card number (see our About Us page for addresses, numbers, and email addresses).
